Regarding: 0% motility in a 43 year old male with
sarcoid but otherwise healthy and has a 14 year
history of primary infertility. Semen volume,
concentration are normal, % normal sperm is at lower
limit of normal. No evidence for significant
pyospermia. Motility has ranged from 0-10%. Last two
semen analyses showed zero motility. HOS test was
consistent with zero viability. TRUS done in past is
reportedly normal. They will not use donor sperm. Is
there any additional evaluation that would be useful?

Response: How many days abstinence does the patient
have on each specimen? He is using correct procedures
and technique for collection of an acceptable specimen
into an approved specimen container? You might try
ejaculations closer together to see if you can obtain
at least a few motile sperm with which to do ICSI
since he has produced motile sperm previously. If
genetic testing is normal and/or the patient is not
exposed to some toxic environmental factor, you might
consider MESA or TESA/TESE.
