I would appreciate your thoughts on this case.

43 year old male with sarcoid but otherwise healthy. He has a 14 year
history of primary infertility. He had a varicocele ligation in 1990.
Semen volume, concentration are normal, % normal sperm is at lower limit of
normal. No evidence for significant pyospermia. Motility has ranged from 0
- 10%. Last two semen analyses showed zero motility. HOS test was
consistent with zero viability. TRUS done in past is reportedly normal.
Wife is 40 years old. They will not use donor sperm. Is there any
additional evaluation that would be useful?
