Dear Colleagues,
I would like to know your opinions as regard this case;

Male patient 29 years old married since 2 years complain with primary
infertility. He has a normal body configuration, normal secondary sexual
character. Also, he has a normal size of the testis and no varicocele.
His past history is irrelevant. Semen analysis was done to find sever
oligospermia, sperms found only after centrifugation. Hormonal assay was
done to find that;
Serum FSH 4.0 mIU/ml (normal 1.5 - 14 mIU/ml)
Serum LH 4.9 mIU/ml (normal 1.5 - 8 mIU/ml)
Serum testosterone 8.5 ng/ml (normal 2.7 - 17.3 ng/ml)
Serum prolactin 22.5 ng/ml (normal 2.5 - 17 ng/ml)
Serum estradiol (E2) 118 pg/ml (normal ND - 55 pg/ml)
A course of Clomid was taken for 3 months. Monthly semen analysis was
done to find that the number of the sperms is 3 - 5 million /ml.,
motility after 2 hours 50% and abnormal forms account for 50%. Pus cells
5-8/HPF. The patient stopped Clomid for another 3 month, and semen
analysis was done to find that the number of the sperms is 8 million /
ml. and the motility after 2 hours is 60% and abnormal forms account for
50%. pus cells 10-15/HPF.
Now what is the explanation of this case and what are the investigations
and the treatment required.
