The patient described by Dana Ohl indeed may have developed intermittent
allergic symptoms resulting from intermittent sperm leak from the testicular
end of the vas following his vasectomy. No other feasible explanation seems
apparent. Assuming the supposed etiology of his symptoms to be correct, I
would be reluctant to suggest bilateral epididymectomy because of fear of
causing a disruption of the blood-testis barrier by creating an obstruction
so close to the testis (by performance of epididymectomy). Again assuming
that the explanation for the symptoms is a true sperm allergy, I would be
much more comfortable recommending vasectomy reversal for this particular
patient----with the understanding that it may not relieve his allergic
symptoms. I shall be interested in the thoughts of other contributors to
Androlog concerning this unusual situation.
