Designated Donor

I am preparing to bank sperm from a designated donor for the first time, and
I am developing policies and preparing consent forms.

I have the following questions and would be interested in hearing the
policies of other sperm banks regarding designated donors.

1. I will have the designated donor undergo all of the disease screening
that I use for regular donors, and will quarantine the semen as usual.
However, should the designated donor also undergo genetic screening by a
medical geneticist (as my regular donors do)? Or should I consider this as
being similar to husband semen in that regard (i.e., we don't require that
client depositors undergo genetic screening, but we do require disease
screening if the semen is to be used for a recipient not his wife)?

2. Can the designated donor choose to have his banked semen destroyed at
any time? (e.g., even if the designated recipient does not agree)?

3. If the designated donor is married, should consent be obtained from his
wife?

4. If the designated donor dies, can his wife choose to have his banked
semen destroyed?

5. Can the designated recipient choose to have the semen shipped to another
facility without the designated donor's consent?

6. Can the designated recipient choose to have the semen destroyed without
the designated donor's consent? (e.g., if she decides she doesn't wish to
continue paying the storage charges)?

7. Can the designated recipient choose to donate the semen to another
designated recipient or to the sperm bank for research without the
designated donor's consent?
