I agree with Arnold Belker and Bob Oates that the fructose test is usually
not important if the semen volume is normal, especially if it comes out
clumpy and appropriately liquefies. I can imagine some situations in
azoospermic men with a low volume, a positive or equivocal fructose might
suggest an obstructed ejaculatory duct or partial aplasia of the vas on one
side and an epididymal or iatrogenic vasal injury contralaterally. I also
think the post-ejaculate urine is unnecessary in men with absolute
azoospermia in the antegrade ejaculate (unless there is no antegrade
ejaculate). I have never seen a man who was absolutely azoo in the
antegrade have anything in the urine.
